This Privacy Policy aims to break down how Physiolosophy collects, processes and stores the Personal Data you provide to us via Direct Interaction and Automated technologies. At Physiolosophy, we always respect your Personal Data and treat it with the highest confidentiality. We may change our Privacy Policy occasionally so, please check this page every so often to ensure you are made aware of and happy with the changes.
HOW DO WE COLLECT YOUR DATA:
- Name and contact information. We may collect your first and last name, title, date of birth, email address, postal address, phone number and other similar contact data.
- You will be asked to complete our New Client Information Form when you attend your initial session with us.
- Cross communications with other multidisciplinary professionals or insurance companies involved, i.e. invoicing, referral letters and treatment reports.
- Physiotherapy records are documented on paper and stored in a secured lock cabinet.
- Any personal information you provide us with in correspondence with you.
- We may collect other online identifiers, including cookies information (for more information, please see the section Cookies Policy, the internet browser and devices you are using and the pages you visited on our website and how long you visited us for.
- Your I.P. address (or Internet Protocol Address). This is a unique address used by computing devices such as personal computers, tablets, and smartphones to identify themselves. An I.P. address is analogous to a street address or telephone number and could therefore be used to identify you.
You have choices about some of the personal information we collect. When you are asked to provide personal information, you may decline. Please note that if you choose not to provide personal data, that is necessary to enable us to carry out your request.
We may provide links via Physiolosophy websites to other websites, or you might independently visit the website of a third party who provides services on our behalf. The privacy practices of these third-party websites are outside our control. - Existing or previous medical history
- Private medical insurance details
- NHS Number (in case G.P. contact is required)
- Bank account details (should payment have been made, i.e. deposit to secure your booking)
- Transaction details (payment details of products or services you have purchased from us)
- Technical data, including how you have interacted with our website, devices use, internet provider and other similar data.
HOW DO WE USE YOUR PERSONAL DATA:
- We are required to use your Personal Data to enter a contract with you to deliver the most convenient services and treatments for you.
- Your Personal Data can be required to comply with a legal or regulatory obligation including Contract, Consent, Legitimate Interest and Legal Obligation.
- We may use your Personal Data solely for marketing purposes of the Phyisolosophy services and occasional offers. If you do not wish to receive this information, you can opt-out when filling in the New Client Information Form in your first session.
- Provide you with medical diagnosis and treatment.
You can withdraw your consent at any time by contacting us using the details provided within the section below ‘Your Right to Withdraw Consent To Processing Of Personal Information Or, in relation to any marketing messages you receive, by using the unsubscribe option included in those messages.
We use profiling and screening techniques to ensure relevant, timely communications and provide our supporters with an improved experience. Please see the section ‘Profiling’ for further information. Please let us know if you do not want us to use your personal information this way.
Processing necessary for us to promote our business, products and services and measure the reach and effectiveness of our campaigns
We may send you marketing information occasionally after you have purchased a service from us or enquired about other details of interest in a business context. We will only contact you with information about our services and any additional information we believe may interest you (and in ways the law allows), which we hope you will like. We may also contact you occasionally with marketing information (unless you object). In relation to any such information we send by email, we will include an option allowing you to object to receiving future messages by unsubscribing.

CONSENT OF PERSONAL INFORMATION TO BE WITHDRAWN:

In that case, you can withdraw your consent for that particular processing anytime. Please note that if you ask us to stop sending marketing information, we will update our records to prevent further mailings as quickly as possible. However, you may still receive other mailings already in progress before you asked us to stop for up to 2 months.
The withdrawal of your consent will not affect the lawfulness of our processing based on your consent before you withdrew your support.

PROFILING:
Profiling is often used in direct marketing and involves analysing data to improve communications targeting.
